    Maybe its too difficult to identify the individuals responsible and or
    Nth American laws don't make CEO's or other senior management accountable?

    I just searched on au Amazon and found it but I had to include the book
    title and the authors name, didn't work with just the book title.

    I can't think of any situation where anyone has ever been
    jailed as a result of fucking up on aircraft maintenance.

    I'm not in favour of jailing people for making mistakes. Doing so would probably have the paradoxical effect of discouraging the most able from
    doing the job, because they'd be most aware of how things can go wrong.

    But some of the practices at Alaska Airlines certainly bordered on
    criminal negligence. Maybe there was no way to tie that to the specific
    crash. Though reversing the mechanic's decision to replace the
    jack-screw was clearly highly questionable.
